All claims can be reported directly to the insurance company by using one of the following contact numbers listed on the document attached. Claims may also be sent, by the agent, to Halcyon for reporting. Please submit ACORD claims forms via fax. Lawsuits or claims with large amounts of documentation should be sent by overnight carrier.
