All required items may be e-mailed to tharrington@halcyonuw.com or via fax to 407-660-0525. Halcyon's appointed agent partners have the option of receiving their commissions via ACH Direct Deposit. If you would like to submit this form with your application packet, you may do so by filling out this form.

If all of the required six documents below have been received, your agency's appointment application will be reviewed and a decision made at our earliest convenience. We appreciate your interest and look forward to possibly working together in the near future.
